What is HOT

Not Just
Replacement —
Optimization.

Testosterone Optimization Therapy (TOT) goes beyond simply bringing your levels back into a "normal" range. It's about finding your personal optimal level — where you feel and perform your best.

Testosterone peaks in your 20s and declines roughly 1% per year after 30.

With the right protocol, monitored closely by your Arrow Health provider, you can restore energy, libido, muscle mass, mental clarity, and overall quality of life.

Signs of Low Testosterone

Do Any of These
Sound Familiar?

Low testosterone — clinically called hypogonadism — is often dismissed as "just aging." But these symptoms are real, measurable, and treatable. If you're experiencing several of these, your levels may be worth checking.

Chronic Fatigue

Persistent tiredness that doesn't improve with rest. Low energy despite adequate sleep.

Reduced Libido

Decreased interest in sex or difficulty maintaining arousal that feels out of character.

Muscle Loss

Difficulty building or maintaining muscle mass despite regular exercise and good nutrition.

Brain Fog

Trouble concentrating, forgetfulness, or a persistent sense of mental cloudiness.

Mood Changes

Increased irritability, depression, or anxiety that doesn't have another clear explanation.

Increased Body Fat

Unexplained weight gain, especially around the midsection, despite diet and exercise.

Risks & Considerations

What You Need
to Know

Testosterone therapy is safe and well-tolerated for most men when properly managed. Understanding the risks helps you make an informed decision — and is why ongoing monitoring with your Provider is essential.

Monitor Closely

Erythrocytosis

TOT can increase red blood cell production (elevated hematocrit). Left unchecked, this can raise clotting risk. Regular CBC labs and occasional therapeutic phlebotomy manage this effectively.

Monitor Closely

Testicular Atrophy & Fertility

Exogenous testosterone suppresses LH and FSH, reducing natural production and sperm count. For men concerned about fertility, hCG co-administration can help preserve testicular function.

Manageable

Estradiol Elevation

Testosterone can convert to estrogen via aromatase. Elevated estradiol may cause water retention, mood changes, or gynecomastia. Managed with aromatase inhibitors (AI) if needed.

Manageable

Skin & Hair Changes

Increased oiliness and acne are common, especially early in treatment. Hair loss can be accelerated in genetically predisposed men due to DHT conversion. Often manageable with adjustments.

Low Risk

Sleep Apnea

Testosterone may worsen pre-existing sleep apnea in some men. If you have sleep apnea, it should be treated and monitored during TOT. Not a contraindication for most patients.

Low Risk

Cardiovascular Considerations

Current evidence does not support a causal link between properly managed TOT and heart attack or stroke in healthy men. Monitoring lipids and blood pressure is standard practice.

Important: This information is educational, not medical advice. All Arrow Health protocols are overseen by licensed Nurse Practitioners. Your individual risk profile will be assessed at your consultation. Do not start or change hormone therapy without medical supervision.

Key Tests We
Monitor

Total Testosterone
Optimal: 600–1,000 ng/dL

The primary marker. We look for your personal optimal range — not just "normal" — based on symptoms and how you feel.

Free Testosterone
Optimal: 15–25 pg/mL

The biologically active fraction. You can have normal total T but low free T — often the real culprit behind symptoms.

Estradiol (E2)
Target: 20–40 pg/mL

Testosterone converts to estrogen. We keep E2 in balance — too low is as problematic as too high.

Hematocrit / CBC
Target: below 52%

TOT can raise red blood cell count. We monitor closely to prevent polycythemia and manage with phlebotomy if needed.

LH & FSH
Baseline diagnostic

These pituitary hormones help determine if low T is primary (testicular) or secondary (pituitary/hypothalamic) in origin.

PSA
Baseline + annual

Prostate-specific antigen is monitored as a safety marker. TOT does not cause prostate cancer but we screen proactively.

Frequently Asked Questions

Everything You
Want to Know

The only way to know for certain is a blood test. We measure total testosterone, free testosterone, and other related hormones. Clinically, levels below 300 ng/dL are considered low — but symptoms matter just as much as numbers. Some men feel terrible at 350 ng/dL; others feel fine at 280 ng/dL. Your Arrow Health Provider evaluates both your labs and how you feel to determine if treatment is appropriate.
When properly managed with regular monitoring, testosterone therapy has a strong long-term safety profile for most men. Large-scale studies — including the TRAVERSE trial — have not shown increased cardiovascular risk in men without pre-existing heart disease. The key is consistent lab monitoring (every 3–6 months), dose management, and working with an experienced provider who adjusts your protocol as needed.
Not necessarily — but many men choose to continue long-term because the benefits are meaningful. When you start TRT, your body's natural production typically decreases (due to suppressed LH/FSH). If you stop, levels usually return to pre-treatment baseline within a few months, though recovery time varies. We can also use PCT (post-cycle therapy) protocols to support recovery if you decide to discontinue.
Yes — exogenous testosterone suppresses sperm production in most men by reducing LH and FSH, the hormones that signal the testes to produce sperm. This is reversible in most cases after discontinuation, but it can take months to a year or more. If you want to preserve fertility, we can add hCG (human chorionic gonadotropin) to your protocol, which mimics LH and maintains testicular function and sperm production while on TRT.
Most men notice improvements in energy, mood, and libido within 3–6 weeks of starting therapy. Body composition changes — more muscle, less fat — typically take 3–6 months to become clearly visible. Full optimization, where your protocol is dialed in to your ideal levels, usually takes 3–6 months of monitoring and adjustment. Everyone responds differently, which is why ongoing lab monitoring and check-ins with your Provider are essential.
Anabolic steroids used in bodybuilding typically involve supraphysiological doses — far above normal testosterone levels — often stacked with multiple compounds, without medical oversight or monitoring. Testosterone Optimization Therapy aims to restore your levels to a healthy physiological range, with careful monitoring of blood markers, cardiovascular health, and hormone balance. The goal is long-term health and wellbeing, not maximum muscle mass.
Arrow Health operates on a direct-pay model — our visit fees are transparent and affordable ($100 first visit, $150 follow-up HOT visits). This keeps costs low and removes insurance barriers. Prescriptions vary in cost depending on the delivery method: testosterone cypionate injections are often very affordable at $20–$40/month through pharmacies, while gels cost more. Labs through Quest Diagnostics are $25–$50. Many patients find our total monthly cost lower than insurance co-pays and premiums combined.
We're expanding rapidly and currently serve Texas, Illinois, Colorado, Tennessee, Maryland, Louisiana, Wisconsin, Minnesota, Oregon, Alaska, Ohio, and more. If your state isn't listed yet, sign up for our newsletter and you'll be notified the moment we launch in your area. We're adding new states every few months.
