TOT can increase red blood cell production (elevated hematocrit). Left unchecked, this can raise clotting risk. Regular CBC labs and occasional therapeutic phlebotomy manage this effectively.
Exogenous testosterone suppresses LH and FSH, reducing natural production and sperm count. For men concerned about fertility, hCG co-administration can help preserve testicular function.
Testosterone can convert to estrogen via aromatase. Elevated estradiol may cause water retention, mood changes, or gynecomastia. Managed with aromatase inhibitors (AI) if needed.
Increased oiliness and acne are common, especially early in treatment. Hair loss can be accelerated in genetically predisposed men due to DHT conversion. Often manageable with adjustments.
Testosterone may worsen pre-existing sleep apnea in some men. If you have sleep apnea, it should be treated and monitored during TOT. Not a contraindication for most patients.
Current evidence does not support a causal link between properly managed TOT and heart attack or stroke in healthy men. Monitoring lipids and blood pressure is standard practice.

Prostate-specific antigen is monitored as a safety marker. TOT does not cause prostate cancer but we screen proactively.
